User guide

① Important Notice

Numata Medical Clinic is located 3 minutes away from JR Chuo Line Mitaka Station (North Exit). Our new address is 1-23-3 Nakacho, Musashino, Tokyo

Please understand that your appointment time may not be the exact time you see the doctor. We strive to keep your waiting time to a minimum. However, emergencies and medical situations do happen now and then, and we ask for your patience and understanding if there is a delay during your appointment. 

 

Please note that we provide walk-in services for patients who are not familiar with online bookings, as well as for patients who require emergency medical attention. If you need to see a doctor urgently、please give us a call for an appointment. Alternatively, simply visit us during clinic hours.

Electroacupuncture: for eye fatigue, shoulder/lower back pain, headache (¥3000 separately for the treatment of a hand, a foot, an elbow, or a knee)


Beauty acupuncture: for skin brightening, dull skin tone, dark circles under eyes, and wrinkle treatments

On your screen, you will see “My first time visit” (初診の方)on the top, and “I am a returning patient”(再診の方)at the bottom.

If you are using a computer:

 

On your screen, you will see “My first time visit” (初診の方)on the left, and “I am a returning patient”(再診の方)on the right.

④  Select a subcategory when required

If a subcategory of a service is required, a list of further detailed services will be displayed for you to select. If a subcategory is not required, you will be directed to the next page automatically. 

 

Possible subcategories you may see:

  • Internal medicine (my first visit) 内科(初診)

  • Pediatrics (my first visit) 小児科(初診)

  • Vaccination (2 years old and above) 予防接種(小児)

  • Health checkups for elementary school students 健康診断(一小・インター・その他小学生)

  • Internal medicine (returning) 内科(再診)

  • Pediatrics (returning) 小児科(再診)

  • Health checkups for infants and toddlers under 2 years old 乳幼児健診

  • Vaccinations for infants and toddlers under 2 years old 乳幼児予防接種

  • Health checkups and Vaccinations for infants and toddlers under 2 years old  乳幼児健診+乳幼児予防接種

  • Health checkups (with tests for blood sugar and triglycerides) 健康診断(血糖・中性脂肪の血液検査あり)

  • Health checkup (without tests for blood sugar and triglycerides) 健康診断(血糖・中性脂肪の血液検査なし)

If this is your first visit or if your don’t know your patient number, tick the checkbox.

 

氏名:your first name and last name (e.g. Hippocrates van Kos)

 

ふりがな:if you don't know your English name in Hiragana, simply type your first name and last name again (e.g. Hippocrates van Kos)

 

生年月日:date of birth (YYYY/MM/DD)


 

携帯番号:your phone number


 

メールアドレス:email address

⑦ Check your booking information and tap the confirmation button (予約を確定する)

Two more steps before you complete the appointment booking 

 

  1. Double check the information carefully and make sure it is correct. 

  2. Tap “confirm my appointment” (予約を確定する)  to complete the booking.
